Due to the COVID-19 pandemic, the deadline to send in a nomination has been extended to Monday, May 11, 2020. In order for a nominee to be considered, all application materials, with the exception of the Letter of Recommendation signed by all members of the elected Divan, must be received by May 11.
Each nomination must include the following:
• A fully completed Board of Governors Membership Data Form (attached).
• A signed affidavit (attached) from the nominee indicating his understanding of the duties and responsibilities of Hospital Board Members and his willingness to become actively involved in the Board.
• A letter of recommendation signed by ALL members of the elected Divan. A separate letter is required for each nominee. (NOTE: If any nominee is a member of the elected divan, he is not to sign the letter of recommendation.) *This letter will be required, but does not need to be submitted by May 11th. It will be required later this summer.
Nomination materials should be submitted to my attention at [email protected]. If an application was previously submitted, and included the BOG Membership Data form and the signed Affidavit, there is no need to send the application again.
